1. Who are the important people in your child’s life? Please list their names and relationship to your child (e.g., Michelle - sister, Mary - Grandmother, etc.).
    
2. What names should I call you and the other important people in your child’s life (e.g., “Barb and J.J.,” “Dr. Jones,” “Lt. Smith”)?
    
3. What form of address would you like the children and youth in the program and myself to use when we see you (e.g., sir, ma’am, rank)?
